How To Find The Single BEST Strengthening Exercise For Your Piriformis Syndrome
Strengthening the glutes and piriformis muscle helps many people who experience lower back pain, Piriformis Syndrome (pain deep in the glute), and/or sciatica (sciatic nerve pain). But it also makes many others worse! In this video, I share with you 3 possible strengthening exercise options. Then, I teach you my entire process for determining if these exercises will help you, make no change, or make you worse - so that you know if you should or should not do them. Then, if and once we find a motion that helps- I share with you how I apply the concept of using Movement As Medicine for pain relief. Rather than trying a bunch of random exercises, you'll learn the power behind testing (NOT guessing) and will learn a very systematic and scientific way of using motion for piriformis pain relief that leaves you feeling confident about your treatment path moving forward.

01:32 - Goal at the end of this video...

Centralization of sciatica (sciatic nerve pain) is a sign that your pain is coming from a disc in your lower back. If you're dealing with back, butt, or sciatica pain- chances are you feel all sorts of tender spots. Much of the world believes these to be "muscle knots" or "trigger points" and because of this, many people try releasing them by getting deep tissue massage, foam rolling, using tennis balls and all sorts of other gadgets. Is Your Piriformis Muscle Pinching Your Sciatic Nerve?

It's very rare, accounting for just 6% of glute/sciatica issues according to statistics. Many people have been led to believe that the piriformis muscle is pinching their sciatic nerve, and this belief has resulted in much time, energy, and money attempting to release it- by way...

    So should we just lay in bed 😭? I feel like I’m on a seesaw with this sciatica. Some things that worked before now cause pain. Everyone’s saying stay active. It’s so confusing 😢

    • @drcharliejohnson
      @drcharliejohnson День тому

      Bedrest has not been shown to result in best outcomes when compared to the recommendation of staying as active as possible. It comes back to doing detective work to figure out why symptoms persist (need to consider mental/physical triggers or inputs) and then having a framework for deciding when symptoms are okay and when you're pushing them too far. And then, following a process to determine what specifically will work best for you. No pressure, but if you're interested in some help- I'd be happy to review your case personally. Just visit www.drcharliejohnsonpt.com/apply if you'd like - set up a time to chat with us and then follow instructions to submit details of your case. Hang in there. Dr. Charlie

    My butts hurt, my figure 4 test looks like yours, both legs hurt. Right more than three left. I watched this and I still don’t know what to do to solve my pain. P.s. I am 60 F, yoga several times a week. Walk or hike daily. Average weight. I am sick of being in pain for 5 yrs now. I was diagnosed via MRI that I do have iliopsoas bursitis and found a tight piriformis can cause that. The drs I have seen are useless, PT hasn’t helped.

    This is great info, seriously. I have pain at the right hand side possibly to overstreching to get a ball playing pickleball. I could not get out of bed or put socks on without major pain. I have numbness and tingling that goes down the side of my leg and feet tingling too and pain in the middle and upper glute. MY back does not hurt. I can do the cobra , etc, without pain. I think you mentioned it but I think I have PIRIFORMIS Syndrome. What you were not clear about or I missed it was whether an inflamed piriformis will affect the sciatic nerve. GREAT STUFF. p.s The straight seated leg test hurts.

    Piriformis exercises like the figure 4 pulling up the knee seem to cause me back problems so a massage gun may be helpful . I watched with interest. Thanks. I used your diagnostic sheet and am pretty confident my flare ups are SI joint related. I have been doing bridges, clams and side leg raises as instructed by my PT but I still get flareups ...often after doing these as part of a yoga routine.I try to include stretch and strengthen for all muscle groups in my yoga practice. I enjoy fig 4 so I do it every day sitting on my bed before getting up. The clam definitely feels yukky to do but bridge stabilising w yoga block between knees feels nice. Leg raises are ok if I don't lift too high. So should I lay off clams for now? I am trying to better understand your methodolgy. I like to understand what I am doing and why. I thought that stretching and strengthening would resolve my issues permanently but I thing SI joint may need constant maintenance, right?

    Hi Dr Johnson, my case is as follows: diagnosed with plantar fascitis and Baxter's nerve entrapment. PF seems more or less gone. X ray showed arthritis in left hip. Got the left hip adjusted and since then have what could be S1 related burning and stinging pain in both heels but worse on the right side, getting worse with walking. No leg pain but ridiculously tight calves and hamstrings. Physiotherapist suspects tight muscles in pelvis could irritate sciatica which I can feel in both feet (heels and inside of feet). I can't walk any longer than 5 minutes, my life is on standby. Physiotherapist did a treatment where he worked on unblocking nerve flow on left side of outer thigh. Felt extreme release of blocked energy and next day the symptoms were reduced by 40%. He told me to keep walking, however I overdid it and now heel pain is 8/10, unbearable. Any chances you may know what is going on with this pain and what I could try to alleviate this ..? Most appreciative and sending you warm regards, thank you for sharing your knowledge!

    I have drop foot on right side and muscle weakness that does not allow me to put weight on that leg. This is due to central spinal stenosis. My understanding is that it takes ten to twelve months for the nerve to heal (if ever). Will the muscles not atrophy if not strengthened during the waiting period?

    • @drcharliejohnson
      @drcharliejohnson 7 днів тому

      Hey Kevin! If nerve function is reduced to the motor units which supply the muscles then you could attempt strengthening- but chances are it will have minimal effect. That being said, my recommendation would be to stay as active as possible in life and exercise. If I may ask- What is the treatment plan to resolve this long term?

    None of those motions make my pain worse. Are there any other test motions?

      @drcharliejohnson 7 днів тому

      For sure- could be any motion that reproduces your pain/symptoms. Lunges, squat, jump, stairs, side bend… I just show some common motions that are often yucky. However- if none of these motions reproduce your pain, this essentially rules out disc, piriformis, hip, or sciatic nerve issue 👍

      @@drcharliejohnson ok, I tried the motions again. Some of them hurt, but they don't make my _back_ hurt, they just hurt in and of themselves. The only one that makes my _back_ hurt is the side leg raise. Most of my pain is central lower back out to right hip, sometimes going down the right leg. The right hip is actually quite painful to lay on, and wakes me up at night. The leg raise on the left (good) side hurts a bit in itself. The leg raise on the right (bad) side makes my lower LEFT back and buttocks hurt! I have a history of disc trouble, and had a micro discectomy on the left side in 2009, which cured the left sided pain.

      @@fiestacranberry hey! Okay got it... so, I'd pick the side leg raise as your yucky motion (so long as it's tolerable to retest... but likely means that the lying right and left side leg raises are NOT options to help you... meaning if they are yucky- don't treat using a yucky motion). If it's not tolerable to perform/or use as your yucky motion to test and retest... then pick the LEFT good sided side leg raise that's still a bit yucky and see if any of these motions begin improving that. Make sense? No pressure, but if you need more direction I'd be happy to review your case to see how I may be able to help. Just visit www.drcharliejohnsonpt.com/apply if you'd like to set up a chat with us- then follow instructions to submit your "Case Review." Okay? Dr. Charlie PT

    For me, bridges 10 times for 10 sec and donkey kicks, 50 each leg.. I also do flutter kicks, 100 and side leg raises, 50 each side

      Is this working for you? If so, awesome… if not- sounds like it’s going to be hard to tell what’s working (many different inputs at once).

    Thank you for sharing Kyle. Doc Charlie is my go-to when I get flare ups. Reminds me about my mental dialogue and also the movements I do that is essentially 'poking the bear'.😊
