How To Find The Single BEST Strengthening Exercise For Your Piriformis Syndrome
Strengthening the glutes and piriformis muscle helps many people who experience lower back pain, Piriformis Syndrome (pain deep in the glute), and/or sciatica (sciatic nerve pain). But it also makes many others worse! In this video, I share with you 3 possible strengthening exercise options. Then, I teach you my entire process for determining if these exercises will help you, make no change, or make you worse - so that you know if you should or should not do them.     I watched with interest. Thanks. I used your diagnostic sheet and am pretty confident my flare ups are SI joint related. I have been doing bridges, clams and side leg raises as instructed by my PT but I still get flareups ...often after doing these as part of a yoga routine.I try to include stretch and strengthen for all muscle groups in my yoga practice. I enjoy fig 4 so I do it every day sitting on my bed before getting up. The clam definitely feels yukky to do but bridge stabilising w yoga block between knees feels nice. Leg raises are ok if I don't lift too high. So should I lay off clams for now? I am trying to better understand your methodolgy. I like to understand what I am doing and why. I thought that stretching and strengthening would resolve my issues permanently but I thing SI joint may need constant maintenance, right?

    Hi Dr Johnson, my case is as follows: diagnosed with plantar fascitis and Baxter's nerve entrapment. PF seems more or less gone. X ray showed arthritis in left hip. Got the left hip adjusted and since then have what could be S1 related burning and stinging pain in both heels but worse on the right side, getting worse with walking. No leg pain but ridiculously tight calves and hamstrings. Physiotherapist suspects tight muscles in pelvis could irritate sciatica which I can feel in both feet (heels and inside of feet). I can't walk any longer than 5 minutes, my life is on standby. Physiotherapist did a treatment where he worked on unblocking nerve flow on left side of outer thigh. Felt extreme release of blocked energy and next day the symptoms were reduced by 40%. He told me to keep walking, however I overdid it and now heel pain is 8/10, unbearable. Any chances you may know what is going on with this pain and what I could try to alleviate this ..? Most appreciative and sending you warm regards, thank you for sharing your knowledge!

    I'm only 36 and my lower back right on my spine and my lower left back is already garbage, and every year it gets worse. I've tried all sorts of stretches, ones I'm flexible enough to do, and nothing works. I'm also super fit because i take care of my health, its very depressing, really. Think I'm gonna finally call my doctor.

      I am so very thankful Dr Charlie, after 2 strenuous bodypump sessions I woke up in the middle of the night in so much pain from my Left butt muscle (went to Remedial massage and was told it was my piriformis, however, also took your test) so long story short was doing youtube STRETCHING exercises which not only exacerbated my injury but gave me no pain relief until a week later came upon your video's........love your work Charlie!!     I have drop foot on right side and muscle weakness that does not allow me to put weight on that leg. This is due to central spinal stenosis. My understanding is that it takes ten to twelve months for the nerve to heal (if ever). Will the muscles not atrophy if not strengthened during the waiting period?

      Hey Kevin! If nerve function is reduced to the motor units which supply the muscles then you could attempt strengthening- but chances are it will have minimal effect. That being said, my recommendation would be to stay as active as possible in life and exercise. If I may ask- What is the treatment plan to resolve this long term?

      For sure- could be any motion that reproduces your pain/symptoms. Lunges, squat, jump, stairs, side bend… I just show some common motions that are often yucky. However- if none of these motions reproduce your pain, this essentially rules out disc, piriformis, hip, or sciatic nerve issue 👍

      @@drcharliejohnson ok, I tried the motions again. Some of them hurt, but they don't make my _back_ hurt, they just hurt in and of themselves. The only one that makes my _back_ hurt is the side leg raise. Most of my pain is central lower back out to right hip, sometimes going down the right leg. The right hip is actually quite painful to lay on, and wakes me up at night. The leg raise on the left (good) side hurts a bit in itself. The leg raise on the right (bad) side makes my lower LEFT back and buttocks hurt! I have a history of disc trouble, and had a micro discectomy on the left side in 2009, which cured the left sided pain.

Okay got it... so, I'd pick the side leg raise as your yucky motion (so long as it's tolerable to retest... but likely means that the lying right and left side leg raises are NOT options to help you... meaning if they are yucky- don't treat using a yucky motion). If it's not tolerable to perform/or use as your yucky motion to test and retest... then pick the LEFT good sided side leg raise that's still a bit yucky and see if any of these motions begin improving that.     I get a alot of burning and tingling sensations in my right foot calf and back of hamstring ONLY when i drive a car for about 5 minutes or so. Have had this for over 2 years and still ongoing 😢. Tried everything you can name and still no joy. I will try this and hope to see some results

    Thank you SO much! I do not currently have health insurance, so I’m unable to go to the doctor, which was the reason for my research. I am blown away at how detailed your diagram is, and how helpful it is. Based on all of the steps, I believe the cause of my buttock pain, radiating down the back of my thigh and sometimes into my foot, is truly caused by SI Dysfunction (upstream). My symptoms began about 9 months ago, when working as a delivery driver for an HVAC company. Eventually, while driving for even 20-30 minutes, my right leg began hurting so bad, to the point I would have to put my hand under my leg for any relief, and at times, I’d have to pull over and get out and walk around to get relief. When I press the SI on my right side, the pain is excruciating. The left side is painful as well, or at least very tender, so it may even be Bilateral SI. I plan to take some anti inflammatory medication to start, and go from there. I am very grateful for this video, and the fact that the certified information is free. Beyond grateful!       Thanks Dr. Charlie. The answer is yes, I would love to have you review a case study on my condition. Given a series of unfortunate events, I currently do not have WiFi, and the service out here in the country is rather weak and unreliable.     hmmm. I can raise my right leg no problem but my right hip is on fire and pain radiates far down my leg. Confusing.

    So glad he’s better. So I heard that many people have this symptoms show up in the MRI. What makes one’s symptoms worse than others? in other words, I have some of these and all of a sudden one day in February I was in excruciating pain, but do not remember the event that led up to it

    Hi Dr. Charlie Johnson, thank you for this video because it saved me from a longer and painful recovery! Now that the pain is about only a 2/10 and for about less than half of the day, how long would you recommend sleeping with pillows? Because now that I use a pillow while lying down, my back seems to hurt more than without it, but I don’t know if that risks me triggering the piriformis muscle again.

    10 years in for me. I was a personal trainer and loved free weights. It started when the Dr told me i was hyperflexive. I was a pilates junkee. After birthing 4 babies my relastin and ligaments are trashed. Lo and behold i have flat feet, flat back and 3 anular tears and SI issues. DDD and some calcification spurs on some. 5 herniatons in 4 years. Extremely painful. You literally feel the torn discs. And are we not seposed to fear our movements? After my 4th herniation I was determined to get well. Put my diet to raw mostly vegan. Felt inflammation leave after 1month. Started stretching again. Feeling way better. Then month 2 got the flu. My body was aching and something caused my l5 to herniate and the siatica just let up from pain of 10 to 2 after a week. Plus i don't take pain pills. I take a wholistic approach with herbs and teas. Now what? Im literally terrified to move. Are we not seposed to floss the siatic nerve?

    I did have bad sciatic pain, then it went and I was left with a limp. I’m assuming it’s an issue with the S1 because I can’t go up onto tiptoes on my left foot but can on my right. I can’t even lift it at all.
